Use this official form to refer one candidate for one specific CB5 position. The award for that requisition must be published by CB5; the award tiers are $1,500, $2,500, $4,000, and $5,000.

How an award qualifies

Obtain the candidate’s permission first. CB5 will confirm the candidate names one referrer and check its records for prior contact within 12 months. If hired for the referred position, the candidate must complete 90 consecutive days of work before an award vests.
